We are committed to provide the best possible care. We are happy to help you receive your maximum allowable benefits with your health insurance plan. In order to achieve these goals, we need your assistance and your understanding of our payment policy.

Co-insurance and deductible amounts are due at the time of visit. Failure to make those payments at the time of your visit will result in a service charge to your account. We accept cash, check, MasterCard, and Visa. We will be happy to help you process your insurance claims for reimbursement by providing an itemized statement with all necessary information your insurance carrier needs for prompt reimbursement. Columbia Medical Associates, PA, also participates in a broad variety of managed care/HMO/PPO Programs. If our office does not participate with your insurance carrier, please remember that;

• You are responsible for all charges to your account;
• Some "routine" services such as preventative medicine may not be covered by your carrier; and
• Most policies have a deductible/co insurance amount.

We accept standard Medicare and most private insurance plans. Please contact your insurance carrier prior to scheduling/confirming your appointment to verify that our physicians are listed in with your insurance carrier. It is your responsibility to provide all current insurance and billing information and notify us of any change in your insurance status or mailing information. You will need to update your insurance signature annually or anytime it changes.

Billing statements are mailed monthly. Account balances 90 days past due are considered delinquent. However, we realize that temporary financial problems may affect timely payment of your account. If such problems do arise, please contact us promptly for assistance, otherwise, accounts over 90 days are placed with our collection agency and your care may be terminated with our practice.
